I hope someone can point me in the right direction.
I would like to put "Where did you hear about us?" on my website. I have found the Manage Dynamic Form Fields area and I can add to the boxes. Can someone tell me how I then turn this feature on so it is seen in the check out?

I think what you want to do is go into Admin > Orders & Payment > Manage Order Form Information Fields. Update a custom field setting it's Display Value to "How did you hear about us?" and set Display Field on Order Form to "Display". Then set the Form Field Type to Single Selection List (or Radio Buttons, whatever) and set the Option Values to the choices you want them to have -- Search Engine, etc. It should show up on the first page of checkout.
The Manage Dynamic Forms creates a whole page you can link to -- like for a survey or something I guess. Like the Feedback page, the results would then be emailed to you separartely from orders.

That worked great! Thank you. I have one more question. I would like to change the order that they are in on the check out page. I am guessing that is one of the .csv files. Can you tell me which one it would be in?

Glad it worked for you. The choices that show up in your custom field are in orderaddsel.csv. If you want to change where the field shows up on the checkout page, that's a whole different matter.
