Default Error Carrier

I use USPS for real time postage calculation. When a single item is purchased, all goes well. When a multiple number of same item is purchased, I get a Default Error Carrier message and the default postage is used.

I can have multiple items purchased individually, and postage is calculated fine. When multiple items are purchased, I get the Default Error Carrier message.

Any ideas what could be wrong?

Here are 2 items that generate the error:

Item 1:  Length: 12,  width: 12,  height: 2  weight: 1
Item 2:  Length: 12,  width: 6,    height: 1  weight: 0.5

Shipping Settings (Settings)

Max combined weight: 15
Realtime Min Box Height: 2
Realtime Min Box Length: 12
Realtime Min Box Weight:  0.5
Realtime Min Box Width:  6

I have some more information. Still need help!!!!!!!!

I believe I have found part of the problem.  The only selection I have is First Class in the Checkout process. When my weight exceeds First Class, then I get the default error.

When I ship an 8oz (.5 lb) package all goes well and it shows First Class postage. However I do not get other options.

In REALTIME SERVICES is set as "True" :  First Class, Express Media, Parcel Post, and Priority Mail

What am I doing wrong?

I reran the 7.1.3 upgrade Installer and all is well now. Thanks for help
